Policy and contract specific Customer Service and Support forms

Some of the forms clients search for on Equitable.com – Required Minimum Distribution (RMD), Assign a Beneficiary, etc. – are actually specific to an individual’s policy or contract and require account holders to login to their account to access.

Once logged in, open the "Manage your account" option on your dashboard and click on the “Forms” link in the middle “View” column. This should take you to the “Service Forms” page, which will display all the forms specific to your account.
